Reporting Requirements
The Lincoln Trail District Health Department is responsible for the prevention and control of infectious and communicable diseases in our 6 county area. Staff will provide investigations of the diseases to determine the source of the disease/illness. We also determine whether other individuals need to be contacted for evaluation.
Surveillance of diseases/illnesses may help determine things such as the following:
- Estimate the size of the health problem
- Determine where an illness is occurring geographically
- Detect epidemics or define a problem
- Detect changes in health practices
- Facilitate emergency planning
Kentucky Law 902 KAR 2:020 requires medical providers, hospitals, and laboratories to report infectious and communicable diseases to the local health department serving in the jurisdiction the patient resides.
Report Infectious Disease To Health Department
To report an infectious disease/illness to the health department, please complete the required form and submit the report as indicated. All non-COVID communicable disease reports being submitted to the Lincoln Trail District Health Department per 902 KAR 2:020 will need to be submitted to a new fax number. The fax number for reporting all communicable diseases including COVID-19 is 270-209-0403.

Submit to the Local Health Department in which the patient resides.
HIV/AIDS Cases:
EPID 200 Form
